Running 'db_recover' solved this problem, now slapd stopping/starting working fine without any warning or errors.

While you're at it. why not upgrade your OpenLDAP version to 2.3 (2.3.34 is the latest version, though 2.3.32 is classed as "stable"). This will do your db_recover automatically, though you shouldn't be getting this problem with a properly built db and up-to-date bdb libraries. 2.3.32/4 has a number (like tons) of significant improvements over previous minors.
