I see you are using LDBM. LDBM is a known problem database. I would highly advise you to export your current database, and upgrade to one of the modern backends like bdb.

What sort of known problems are there in LDBM? I had a quick look through the issue tracker but couldn't see anything LDBM specific; are there any resources you could point me to with more info?

There is a reason it was removed from OpenLDAP 2.4.
