Thanks for the pointer. After walking away from the problem for a few 
hours and discerning the detail in what you said I changed this in the 
slapd.conf file

database    bdb
suffix        "o=JNDITutorial"
rootdn        "o=JNDITutorial"

and changed this when running ldapadd:

ldapadd -x -D "o=JNDITutorial" -f tutorial.ldif -w secret

Which I have sent in as the solution in case someone else finds 
themselves up against the same problem.
