Have you run a db_recover?

Also, I'll note that OpenLDAP-2.1.12 is very old and buggy.  You should 
consider upgrading to 2.1.22.  BDB is also at 4.1.25, you probably want to 
upgrade that as well.

You may wish to read the docs at http://www.sleepycat.com/ for more 
information on the BDB errors you are seeing.
