How to keep history of changes

Hi,

In your experiences, what could be a good way keeping history of
changes made to the data, for auditing purposes?

For example changes on a person entry 'xxxAccessCardNumber' attribute.
Is there a known practice to get this information in a sort of RCS/CVS
like manner?

I am open to any suggestion. Especially if you have deployed a system
using OpenLDAP, which solves same kind of history/audit problem.
