has bind_simple_unprotected been removed from servers/slapd/config.c
because there are other recommended ways to prevent unprotected simple
binds with out tls (security clause seems to be a start)? this is with
2.1.12 and the man page still references bind_simple_unprotected as a
valid feature for the disallow clause
