I haven't been following your posts; have you posted debugging information
to the list? I'd suggest trying various debugging levels; sometimes the
debugging output will call your attention to something simple that you're
hitting on.

% directory       /usr/local/var/openldap-ldbm

This may be a silly question, but does this directory exist (and is it
writable by the user you run slapd as)?
