Do you have included these in slapd.conf ? :
# some necessary LDAP-schemes
include /usr/local/etc/openldap/schema/core.schema
include /usr/local/etc/openldap/schema/cosine.schema
include /usr/local/etc/openldap/schema/inetorgperson.schema
