Both naming forms guarantee uniqueness through registration.
The former is based upon your domain registration, the latter
requires registration with your national authority, ANSI in
the US.
You can use unregistered DN if you wish, but like pulling
domain names out of your ass, they won't be of much use in
the global arena.
However, the real advantage of Domain based naming the
ability to do domain based service location. That is,
dc=domain,dc=com can be mapped to domain and this can be
used to locate the directory service holding
dc=domain,dc=com DIT.

You can register your domain under .us if you want both
geopolitical naming and domain based service location.

There are many good MTAs and IMAP/POP servers with integrated
LDAP support. They are have their pros and cons and the best
choice depends on many factors. It's likely your existing
MTA supports LDAP, I'd start there.
