I don't know if it supports LDAP out-of-the-box, but NOCOL seems to be
reasonably popular for monitoring applications/services:

It'd be nice if OpenLDAP had built-in support for SNMP monitoring of the
directory MIB (a la Squid's built-in SNMP support);  something else to add to
the -devel to-do list I guess...
