obsoleting LDBM

I have updated various slapd(8) related manual pages,
as well as some release documents, to either avoid mention
of LDBM all together, or when necessary, to indicate the
backend is obsolete in favor of BDB and HDB.

I don't have time at the moment to do same in the
Admin Guide.  If you do, please feel free to submit
a patch against the HEAD version.  (Please speak up
on this list if you choose to do these edits so as
to avoid duplicating work.)
