OK, then it's already there: back-monitor provides the capability for
back-monitor aware modules to register attributes, entire entries and
even callbacks into itself.  We used this to provide a LDAP-based
management interface for a very sophisticated overlay.  See the
monitor_back_register_*() stuff in back-monitor/init.c; but slapd needs
to be running to see those entries.
