2.1 loose ends

Here is a short list of 2.1 TODO items:

- Man pages (especially section 3)
- Update Admin Guide
- Complete back-bdb ACL Group/Attribute support
- Add configuration knobs to enable/disable various SASL features
- Add SASL(db) Modify Password support
- Specify SASL authorization schema
- Specify Monitor schema
- Fix test011-subtree-repl portability issues
