If an index was configured in slapd.conf but not created yet (e.g. slapindex
wasn't run) then slapcat will fail because it tried to open the index DB in
read-only mode and it doesn't exist yet.

slapcat shouldn't even bother to open the index DBs since it doesn't use them.
Fix coming shortly.
