It is totally unrelated to DB_CONFIG; it's related to ACLs: if I replace
access control with "access to * by * write" it works just smoothly.

I've turned your case into a regression test.  To replay, get this
tarball

<www.sys-net.it/~ando/Download/regressions.tar.gz>

untar it from within the tests/ directory, then execute

./run ../data/regressions/its4184/its4184

sorry for the unfriendly path, it's not intended to be executed by
humans but rather to become an automatic regression check.
