I've committed a subset of your suggestions to the HEAD
branch.  The other changes require additional review.

In the future, please provide diffs in unified format
([cvs] diff -u).  Also note that it's best to make
and report changes relative on one branch, preferably
the HEAD branch.  (You appear to have made changes
on OPENLDAP_REL_ENG_2 but provided changes were
versus HEAD, this often results in patches that revert
changes between branches).
